For us, community care giving begins with YOU!  Our dedicated professionals will meet with you
to help design a care plan that fits the needs of your family.

What comes after the diagnosis?
Most people who are faced with a diagnosis of dementia, Alzheimer’s Disease or other forms of memory loss or
brain trauma simply don’t know where to turn! COA can help you take that all-important next step!

COA’s Community Care Giving is a free, confidential service for St. Johns County residents and their families.
Our qualified dementia care specialist will:

* Set up a time to discuss your situation one-on-one
* Develop an individualized education and needs-assessment
* Offer support and guidance that will empower you to implement solutions for your challenges.

Find Caregiver Support in Your Neighborhood

The Council on Aging offers Caregiver Support Meetings on the following days and at the following locations:

River House, 179 Marine St.
1:00 p.m. on the 2nd and 4th Thursday of every month

Ponte Vedra Memory Care Center 1048 A1A North Ponte Vedra, FL 32082

12:00 p.m. every Monday (except holidays)

These meetings are open and all caregivers are encouraged to attend.
