Comprehensive in-home care options designed by our professional COA care team. All services implemented in the home are provided by our certified COA home health staff.
Independent Living Services
Provide community-based services that facilitate in-home living for those over 60 years of age.
Care Connection
A volunteer-based community outreach program that offers a variety of no-cost services to allow seniors to continue living independently and in their own homes.
Legal Aid
No-cost legal advice for those over 60 through a partnership with St. Johns County Legal Aid.
Prescription Assistance
Facilitates access to low-cost or no-cost medications for eligible St. Johns county seniors. This program has saved elder residents over $1 million dollars.
Information and Referral
A member of our professional service team will help guide you to the services you need for yourself or your loved ones. By answering just a few questions, our COA staff can begin a process to address your needs or, if necessary, direct you to the appropriate agency.
